Insurance Plans

Insurance plans vary widely in the amount they pay for office visits, laboratory tests and procedures.  Review your insurance plan carefully so that you have a good understanding of your benefits.  Most insurance companies have a toll-free number on subscribers' cards for help with questions about coverage.  Our office will cooperate fully in assisting you so that you may receive your maximum insurance benefits.  We participate in most insurance plans offered in the Chapel Hill area.  If you have an insurance plan in which we do not participate, we will provide you with a completed claim form so that you can file for your benefits.

Please remember to bring your insurance card to each visit.  Deductibles and copayments must be paid at the time of service, and old personal balances must be paid before scheduling future appointments.  Some insurance plans do not pay for preventive health care, including physical exams.  If you discover that your insurance company has rejected a claim based on your coverage, we cannot change the coding of the service performed in order to get the claim paid, as this is considered fraud.

If you participate with a high-deductible health plan, you will be required to pay a partial payment of $75.00 at the time of service.  We will also require a copy of the health savings account debit/credit card or a personal debit/credit card to remain on file so we may charge for any additional patient responsibility.  We will send a receipt if your account is charged.  If a card is not kept on file, we will charge for the visit in full and send a refund if your deductible has already been met.  There are addenda to this financial policy, which are signed separately, and available in the office.
