When your child is seen for his or her annual well-child check-up, please ask to have a generic sports form filled out and an immunization record printed at this time. Even if your child has not voiced an interest in playing sports at that time, collect the forms; children often change their minds over the course of a year! Then when you need to give the form to the appropriate organization, make a copy. Keep the originals in your possession!

If you need to have the office fill out a sports or camp form, please allow 5-7 days. During high volume illness seasons (Dec-Mar), please allow 7-10 days. Please note that there is a $10 per  camp form fee, with a $30 family cap. (There is no fee for school sports forms.)
