It is the policy of Chapel Hill Pediatrics and Adolescents to complete forms within 5 business days in order to ensure accurate completion, whenever possible by patient’s regular doctor. 

Chapel Hill Pediatrics will complete forms for patients who have had a complete physical exam/well child check within the last 12 months. 

Families may request that a general form and immunization record be completed at time of annual physical.  This can be kept by family and used for many camp and other activities.  (Note: some organizations will require that patients have their specific physical form completed.)  Please be sure to bring the completed form(s) to the front desk so we copy and save them in our records.

There is no charge for forms completed for daycare attendance, school attendance or school athletics completed in 5 business days or at the time of the physical exam. 

There is a $10 charge for all camp forms completed in 5 business days or at the time of physical exam. 

Families can request that forms be completed sooner than 5 business days.  Chapel Hill Pediatrics will attempt to accommodate these requests.  There is a $30 charge for ALL dropped-off forms that need to be completed in less than 5 business days.
