There will be a charge for all records leaving Chapel Hill Pediatrics.  The entire chart is $8, and a digital, PDF copy is available as well.  You may bring your own flashdrive, and a 1gb drive is also available for purchase in the office for $12.

To request records from our practice, please login to your online account by clicking the "Patient Login" button at the top of the screen and select "Request Medical Records."  If you do not have an account on our website, please call our office so we can send you an account activation email.

Please allow 3-5 business days for our office to process your request and provide you with your records.

If you wish to transfer records from your prior practice to Chapel Hill Pediatrics, please click here to view and print a copy of our Medical Records Release Form.  Complete the form and submit it to your previous practice.
