Since 1960, Chapel Hill Pediatrics and Adolescents has delivered premier health care to infants, children, and adolescents across the Triangle area. With our doctors on the clinical faculties at both UNC and Duke Medical Centers, this two-site pediatric practice has always cared for those children with complex and chronic conditions as well as for those who are mostly healthy. CHPA physicians and staff have spent the past eight years transforming their practice into a true Medical Home -- the ideal health care delivery system for a child/family with more health demands than typical. After representing the State of North Carolina in 2004 at the National Medical Home Learning Collaborative, CHPA charted a course of strategic improvement making changes in their health care delivery. These changes include Care Coordination services, a resource database, customized scheduling to provide adequate time to address patients' complex or long-term illnesses and conditions, and evening, weekend, and holiday office hours. Children with special health needs do best with comprehensive and collaborative health care, which is why CHPA uses their Electronic Medical Record system to communicate with pediatric specialists, to track referrals, and to electronically prescribe for their clients. The physicians, Care Coordinators, and staff work with the family as a team to share information and arrange specialized services and equipment as needed. Pre-visit phone calls to the family help CHPA physicians and parents organize their child's care in advance, improving the efficiency of their doctor's visit. We feel priviledged to care for all children, including private pay and those covered by insurances including, but not limited to, Medicaid, BCBS, Cigna, Duke (BCBS Duke Options and BCBS Blue Care), United HealthCare, Wellpath, Medcost, Champus, etc. The compassionate, family-centered, and coordinated care given by the staff and doctors at CHPA provides an ideal Medical Home. CHPA has been certified as a Patient-Centered Medical Home, with the highest level of certifications by both the National Committee on Quality Assurance and the Blue Cross Blue Shield Bridges to Excellence program. Voted as Best Medical Practice by the Chapel Hill community with the Rose Award in 2008, CHPA was also honored with the 2007 McIver Award for excellence in caring for people with developmental disabilities. In July of 2011, CHPA was selected by Chapel Hill Magazine as the best in the Pediatrician/Family Medicine catagory.
