Physical Exam Forms

If your child needs immunization records, medical records or a complete sports physical, school physical or camp physical, Chapel Hill Pediatrics and Adolescents will provide it! You may request medical records from our office when needed. After completing a child or teen’s physical exam, we also complete health forms required for participation.

We Complete Physical Exam Forms within 5 Business Days

Chapel Hill Pediatrics and Adolescents will complete forms for patients who have had a complete physical exam/well-child check within the last 12 months. It is the policy of Chapel Hill Pediatrics and Adolescents to complete forms within five business days in order to ensure accurate completion, whenever possible, by the patient’s regular pediatrician.

Families may request that a general form and immunization record be completed as part of the annual physical. This can be kept by the family and used for many camp and other activities. Note that some organizations will require that patients complete the organization’s own, specific physical form. Please be sure to bring all necessary form(s), with patient information completely filled in, to the front desk so we may copy and save them in our records.

Charges for Completing Health Forms

There is no charge for completing health forms such as daycare attendance, school attendance or school athletics. All forms will be completed within five business days. For all camp forms completed in five business days or at the time of the physical exam, there is a $10 charge.

Families can request that forms be completed sooner than five business days. Chapel Hill Pediatrics and Adolescents will attempt to accommodate these requests. There is a $35 charge for all dropped-off forms that need to be completed in less than five business days. If our office is unable to complete forms within the time outlined by this policy, there is no charge to the patient.
